Services
Financial Counseling
At the Center for Reproductive Health we feel that
financial considerations should not discourage patients from
receiving the infertility treatment they need. We are therefore
committed not only to offering the highest quality diagnosis and
treatment but also to keeping it as cost efficient as possible. We
want to make your experience at the Center as stress free as
possible and will therefore do everything we can to help you work
through financial matters.
Our financial
counselor will contact your insurance company to verify your
infertility benefits on or before the day of your initial
appointment. During your first visit, she will meet with you to
review your coverage and benefits so that you will completely
understand them. She will give you a copy of your benefits for your
records and will also explain to you what your responsibilities for
payment are, including co-payments, deductibles, and co-insurance.
After your treatment begins, she will continue to assist you,
obtaining
pre-certification of
procedures, testing, or treatment when required and
ensuring that you receive the maximum coverage to which you are
entitled.
Insurance Coverage
The
Center contracts with many insurance companies; however, it is very
important to understand that benefits vary greatly from one policy
to the next. Our financial coordinator is available to help you
with insurance issues, but you can your own best advocate by making
sure that we always have current insurance information. Effective
November 1, 2005, the Center accepts the following plans as well as
many others:

If your plan is
not listed above, call our office to ask if the Center is a
participating provider.
The Center will bill your insurance company if we are
a participating provider. If we do not participate with your
insurance company, we will provide you with the necessary
information so that you may submit claims and obtain reimbursement.
It is imperative that you understand your insurance coverage. You
must review your policy and understand what is covered and what is
not covered. You should review your EOB’s when you receive them and
speak to the Center’s financial counselor when questions arise. We
will do our best to ensure that you receive the maximum coverage to
which you are entitled; however, you must remember that the final
responsibility for your account is yours.
Participating
insurance: Even if you have a participating insurance company,
coverage will vary from one policy to the next. For example, you
may have coverage for infertility testing but not for treatment.
There may be a dollar maximum amount for infertility or a limit on
the number of treatment cycles that are covered.
All managed care
plans (HMO’s) require referral numbers from the primary care
physician before a patient can be seen by the Center’s physician.
It is always the patient’s responsibility to obtain the referral
number—the Center will not assume this responsibility. HMO patients
who fail to obtain a referral number and present it to the
receptionist prior to seeing the physician will be required to sign
a waiver and pay at the time of service or to reschedule their
appointment.
Non-participating insurance: You may have a non-participating
insurance company which provides out of network benefits. If you
have coverage for infertility, your claims will be reimbursed at a
lower percentage, hence making your share of the cost higher than if
you had coverage through a participating insurance company.
Financing
Many patients who
have no infertility coverage choose to finance their treatment
rather than pay out of pocket. However, it is not necessary to
spend your life's savings and sacrifice your financial future to go
through IVF. To help you obtain unsecured financing options with
excellent fixed rates, the Center participates in the IVF Finance
and Support Program, offered by Medical Business Solutions, Inc.
The goal of the
program is to help you preserve your savings and credit for your
future and your child's future. Through guaranteed unsecured
